Clinic Services


At the time of your office visit, we will ask you for information regarding your insurance and payment for services to be provided to you.  We will ask that you present your insurance card to us at check-in so that we can make a copy of it for billing purposes.  Below is a list of some of the insurance companies we participate with.

Aetna, BCBS, Cigna, MTMedicaid, Medicare Plans, First Health Plans, HealthNet, Great West, TriCare, GEHA, Sterling, EBMS, Assurant Health Plans, United Health Care

Audiology benefits depend on your individual insurance plan.  We may be participating providers; however that does not guarantee payment.  Please contact your insurance carrier to confirm that we are included on your plan before your office visit.

Billing and InsuranceIn order for Medicare to cover audiological testing you must be referred by a physician and we must have that written referral on file.  If you have Medicare, please obtain this referral from your physician or have it faxed to our office prior to your appointment. (Fax #:  245-9954)

Other insurances may require a physician referral; please check with your insurance to determine if this is required.

If you do not see your insurance provider and have questions regarding insurance and/or payment options please contact our office.

Financial Policy

  • Payment of all insurance co-pays, deductibles, and non-covered services must be made at the time of service.
  • We will bill all primary and secondary insurance company(s) as a courtesy to our patients. However, the balance due is the patient’s responsibility and payment will be requested from you unless your insurance company pays within 60 days.
  • We will accept payment by cash, personal check and/or credit card (MasterCard/Visa/American Express).
  • Veteran Plans require prior authorization through the VA
  • Workers Compensation plans require claim numbers
  • We have many payment plans available including
    • Wells Fargo 1 year interest-free financing with equal monthly installments
    • In house plans – 90 days same as cash

PLEASE NOTE: Medicare and most insurance plans do not cover hearing aids.  Please call the toll-free number on your insurance card for a description of your benefits.
