Billing & Insurance
At the time of your office visit, we will ask you for information regarding your insurance and payment for services to be provided to you. We will ask that you present your insurance cards and driver’s license to us at check-in so that we can make a copy of it for billing purposes.
Participating Insurance Companies
Audiology benefits depend on your individual insurance plan. We may be participating or allied providers; however that does not guarantee payment. Please contact your insurance carrier to confirm that audiology services are included on your plan before your office visit.
In order for Medicare to cover audiological testing you MUST be referred by a physician and we must have that written referral on file. If you have Medicare, please obtain this referral from your physician or have it faxed to our office prior to your appointment.
